What is Anal Fissure?

Anal fissure is a crack, cut or tear extending up into anal canal from anus causing pain during defecation and afterwards. About 10% of patients going to colorectal surgeons suffer from anal fissures. It may be acute or chronic. It is mainly caused by injury. Trauma is mostly due to injury caused by passage of hard stool thorough anus. Sometimes bleeding fissure can be due to nail of fingers, thermometer, enema tip or endoscope. Episode of diarrhoea or child birth may also be cause of fissure. The skin covering anus and area around is very sensitive; therefore it is very painful in case of injury.

Ninety percent of fissure occurs in posterior midline; however 10% can occur in anterior midline in women. Fissure at places other than anterior or posterior may be due to other causes like malignancy, Crohn disease, infection, HIV etc and need to investigate further.

Anal canal is surrounded by external sphincter and internal sphincter muscles. External sphincter is voluntary control whereas internal sphincter is involuntary muscle. In case of fissure, both muscles are exposed due to injury and get irritated, therefore go into excessive spasm causing pain. Constant pressure on muscle and pulling apart of edges of fissure do not allow fissure heal properly. It is so possible that fissure wound do not heal because of deficient blood supply at posterior midline. Spasm of muscles also may hamper blood supply and prevent healing of fissure.
